Post-mortem

A post-mortem is a blameless review of a completed or failed project that documents root causes and concrete process changes.

Where a debrief looks at the work, a post-mortem looks at the system that produced the work. It is blameless by design: the goal is to find the process gap, not the person.

A post-mortem without a written, owned action item is a conversation, not a post-mortem.
